I’m currently struggling with the same problem and it seems to have existed for several DVDFab versions. I have it with both the current version 13.0.5.4 and version 11.1.0.7 with the profile MKV.Passthrough.
It seems to be due to the time stamp, here on the example of the purchase DVD “Doctor Zhivago” in the 2001 edition. Ripping side A of disc1 works synchronously, but Chapter 1 of side B of disc 1 doesn’t start until about 00:00:14. The ripped MKV file, on the other hand, starts at 00:00:00. The ripped audio track is about 2-3 seconds behind the picture.
The delayed onset of the time stamp can also be observed on other DVDs, whereby the asynchronicity of the ripped audio track varies from a few milliseconds to several seconds depending on the length of the time stamp.
