Despite setting Amazon and HBO Max to download H264, Streamfab v2.0.2.5 is still incorrectly downloading in H265 format and in the case of the new Dune title include HDR, which I do not want. HDR is also disabled in my settings.

I see the same thing.
Set as H.264 and the box for Download 4k unchecked, StreamFab v4.0.2.5 still brings down 1080p as H.265/HEVC from HBOMax, and in the case of Dune it pulls down 1080p H265/HEVC with HDR10.
As a comparison, using a competing product, if h265 1080p is selected you get HDR10. To get non-HDR 1080p for Dune you need to download as H264/AVC.

Its fixed now in 5.0.0.4.
If you go Settings -> VIP Services... and set:
Video Codec (Amazon & HBO Max) = H264
Audio Codec (selected if Available) = EAC3/AC3
Video Resolution = Full HD - 1080p
Download HBO Max videos in 4K HDR/Dolby Vision quality if available = UNchecked
It will download DUNE as H264/AVC, 1080p, no HDR, with 768kb/s 48kHz, 6 channel, E-AC-3 JOC (Dolby Digital Plus with Dolby Atmos)
