Upside Down Fails to copy Error 122

Upside Down (DVD) will not copy. I've tried to copy using "Main Title" copy. It copies to temp folder OK, but then fails to burn to blank disk.
I have also tried making an ISO copy, which completes successfully. However when I try to copy this to disk, it fails and returns "Error = 122".
What does this mean?

Try using different media... Usually is caused by crap media or burning too fast.
