I am having issues creating a Blu-ray copy of this disk on my harddrive. It gets to about 2% and then fails.

you are getting a few read errors.
check the disc for scratches, cracks, blemishes ect
try and clean the disc and try again.

I have tried at least 10 times and each time I clean the disk. There are no scratches, blemishes, or finger prints on the disk.

I also tried ripping the episodes off the disk, DVDFab will only show 11 of the 12 episodes. I was able to extract all 11 episodes without any issues.

I will try borrowing my friends disk and see if I still have the same issue.

That sounds like an error in the disc. Can you identify the 12th episode and play it in a regular BD Player?
