DVD Ripper Recommended file format/size?

Used DVDFab for a few weeks, some general questions:

For Movie Only backup, file storage:
What is most accepted file format (H.264, mp4, .mkv, .m2ts) for a good HIGH QUALITY file.

For the H.264 format, what are most of you setting for size, bitrate, encode pass setting etc?

It seems like the application allows you such control that you can choose anywhere from 2G to a 20G file size for H.264...

I have issues with H.264 streaming PS3 Media Server and FF/RW navigation controls, does not work very well....
