BD Ripper (3D Plus) BD Rip Fail

I have been struggling for a couple of days to have ANY success ripping a BD. I've tried various profiles, but my need/want is just mp4 versions. Tried various new BD discs, DVDFab versions 8228, 9028, 9030, with varying failing results (including no video, choppy video), but the log below shows the most common error when using 9030.

Admittedly, I'm not an expert... just want to archive some of my collection for convenience. Any help and advice would be appreciated...

Here are some of my system specs:

GIGABYTE GA-Z77-DS3H Intel 7 Series Motherboard with 16gb RAM
    Intel Core i3-2100 BX80623I32100 Processor 3.1 ghz
LG 14x Internal BDXL Blu-Ray Burner Rewriter WH14NS40

    22:32:02 Conversion: one pass
22:32:02 CUDA GPU acceleration for video encoding enabled

    22:32:04 Source( 00004.m2ts )

    22:34:33 Work 1 failed, Error = graph 0 error: filter name(native_mux), type(128), id(12), get module_error(module_mux_mp4 module_mux_mp4 error_write_video_sample)
