DVD Ripper DVD ripper recommended format , mpeg-2

Recommendations please for ripping DVDs - which I'm guessing format would be MPEG-2 to ... ?

I do my Blu-ray's to MPEG-4, but when I try the same output preference for DVDs, I either end up with rips that freeze on start, or the audio is quite a few seconds plus off-sync from the video playback.

My guess is I'm making a mistake in trying to "up convert" from MPEG-2 (DVD format) to MPEG-4 (??).

Thanks much in advance as always for any thoughts.
