BD Ripper (3D Plus) DVDfab9 (v9.1.6.6) Crashes reading disc

I have DVDfab9 installed on several machines and working well, however on one particular machine it fails after reading the disk structure. It doesn't matter what disk I'm using, so it's not a disk problem. The BD drive works perfectly with DVDFab8 on the same machine, so it's not a hardware problem.
Other config information:
Windows 8.1 64-bit
Core i7 with HD 4600 graphics (driver version #9.18.10.3165)

I've tried removing and reinstalling DVDFab9 without success. The log is attached.

Looks like the region code might not be set, don't know if that has anything to do with it

Not the region code

I never set the region code, I let DVDfab prompt me, as I often find myself ripping disks from outside my DVD region (in this case I was ripping the Region B release of The Sopranos.) However, it looks like the problem is more systemic, since DVDFab9 will not even start up when the BD player is unplugged, I can't even get to the basic UI.

Never mind

It appears to have been a corrupt graphics driver, after reinstalling the graphics drivers, the problem has gone away.
