How can I rip from DVD/Blu-Ray into multiple formats in one pass? Likely MKV as well as MP4 for iPad.

I think you need DVD Ripper and Blu-ray Ripper to do that.

DVD Ripper and Blu-ray Ripper has similar interface, you just need to change the source to DVD.
The default encoding pass is 1 pass. you can find that setting through "Advance Settings".
