DVD Ripper MP4 will not play

I ripped The Legend of Tarzan into an mp4 and it completed successfully but when I went to play it all players say it does not recognize the file type. I am attaching error files from Quick Time, VLC, and Windows Media Player. I tried ripping a different movie and had the same result.

I was told to tick the disable all GPU codecs check box in Common Settings->A/v codec but that has made no difference. I had no problems using DVD Fab 9. HELP!!!!

You need to post a complete log of one of the failed attempts.

I have this issue once awhile ..... the worst was my new 4K wouldn't play ripped BD, installed Kodi and no problem.

But if the player isn't at fault, I just recopy for a new iso, and then rerip it. if that doesn't solve it I wait for a new version of 'fab and reprocess it. That solves it every time.
