[solved] Task Queue always fails on second file

I'm using passthrough to convert all my .mkv files to .mp4
Every time I choose more than one file to convert, it fails but when I try the file on it's own it converts ok.
It starts the conversion but fails leaving a small 100 mb file.
I'm using the latest version of DVDFab and Big Sur.
I'm guessing there may be some sort of OS permission problem?

    I've found a workaround:

I select a batch of files to convert.
    The first one converts ok.
The rest return an error and leave a 100 mb file in the target directory. (Turn off the error sound to save your sanity!)
I then run the convert script again but this time it asks if I want to overwrite or replace the file.
Choose "Replace existing file" but do not select "Do this for all future conflicts".
I then use Keyboard Maestro to auto click the OK button on a 1 minute loop.
It takes approx this amount of time to passthrough the video and audio to the new .mp4 file.
It's rough but it does work.
Hopefully someone can tell me what option I'm missing to fix this?
    Thanks.
