Sorry for late reply - did not have time to do the work before I went on holiday
Back now and loaded latest 7.0.3.5 beta and did more testing.
Source is Blu Ray Band of Brothers Disc 2, Episode 1 (Europe). This is 1080p/VC1 with DTS HD. DXVA GPU acceleration for video decoding enabled. No Subtitles.
Good news is that A/V sync issues seem to be pretty much fixed although may still be very slightly out (may be 0.5 sec or less) in some situations.
This is slightly strange as they both seem to produce the same h264 format.
Will do more testing and report.

- BluRay to File using mkv.h264.audiocopy is now pretty much perfect as far as I can tell. Great video and in sync audio.
- BluRay to File using mp4.h264.aac has fixed the audio sync issue, but still has choppy/juddery video playback. Seems to miss frames to my eyes. This problem seems to be reapeated with all h264/aac compbinations (e.g. xbox, PS3). This is the conversion I really want to use because of the wide compatibility with many devices.
