I only had a multi-monitor setup once for a short time and it confused the heck out of me. I was adjusting someone else's monitor for them and while I had it I figured I'd give a mult-monitor setup a try. I'll let you take the technical end of this as I'm out of my element.
One thing I would recommend to Sebastian is to get the latest video driver for his graphics card or get on Google and do a search to see if he can find a discussion where other users of that card compare drivers and say which one works best in a multi-monitor situation.

According to your log file, the resolution is 1920 X 1080 with a DPI of 100%.
The second monitor is 1680 X 945. The lowest StreamFab can support is 800 X 600. So, it should be able to display correctly there.
The developer asked how you open StreamFab on the second monitor. By dragging between two screens?
Please confirm whether you have deleted the file SrteamFab.ini. Thanks.
